Patients per Insurance Plan

Purpose:  To list how many patients are enrolled in each insurance plan and display their level of coverage for the plan, e.g. primary, secondary, tertiary.

Note:  This is a "Custom Query Report" (CQR) demonstrating how elligence can create customized reports.
